Cochrane Review Finds Vapes Outperform Traditional Nicotine Patches and Gum

A Cochrane review of 80 studies involving nearly 30,000 adult participants has found that nicotine-containing electronic cigarettes help more smokers quit than traditional nicotine replacement therapy (NRT).

There is clear evidence that nicotine e-cigarettes increase quit rates at six months or longer compared to nicotine replacement therapy, stated Jamie Hartmann-Boyce, lead author of the review and researcher at the University of Massachusetts Amherst.

The statistical analysis shows a 60% increase in cessation success when using nicotine vapes compared to standard NRT options. Nicotine-containing vapes also outperformed nicotine-free alternatives.

These findings align with public health strategies in the United Kingdom, where health authorities utilize e-cigarettes as a smoking cessation tool. In 2023, the British government launched the national "Swap to Stop" scheme, offering free vape kits and behavioral support to up to one million adult smokers.

The harm-reduction approach recognizes that the primary health damage from smoking comes from toxic chemicals produced by burning tobacco, rather than nicotine itself. While nicotine is addictive, e-cigarettes deliver it without combustion.

The authors noted that the review evaluated legally regulated nicotine devices, meaning findings do not apply to unregulated or illicit products. The researchers emphasized that vapes carry health risks and should only be used by adults transitioning away from combustible cigarettes.

The UK is not alone in using smoke-free alternatives to lower smoking rates. Sweden has reduced its daily smoking rate to roughly 5%—one of the lowest in Europe—partly due to the widespread adoption of oral nicotine pouches and snus.
